For individuals with enduring and complex mental health conditions, face challenges in reaching their goals towards rehabilitation, living independently, and engage in their community. Milestone Trust’ two-year programme of community-based rehabilitation, offers a person-centred approach to recovery. The programme focuses on the person being supporting and not on their diagnosis or disability, offering staged approaches and an outcomes-focused pathway to recovery.
Success Factors and Key Lessons
The success of the programme lies predominantly in it’s collaborative structure with the service user. We use a set of core goals and values, and work with individuals to reach goals at their own pace and in their own way. We collaborate with other support systems such as their families, advocates and other healthcare professionals to achieve the best possible outcomes. Since 2024, we’ve expanded our supported housing provision, opening two new step-down services with another in the pipeline to support people with safe and secure accommodation and continuity of staff as they move to more independent settings. We’ve also opened a new service which supports people with acute mental illness as part of the service offering. This has increased the number of people we can support in the community, allowing us to continue to support young people in breaking the cycle of mental illness and hospital admission.

Transferability and Replicability
The values of the programme are based on a human rights based approach that is recognised in all European countries. The model is popular with funding bodies due to the outcomes achieved and the realistic cost of a placement at the service. Since opening in 2020, our budget surgeries have proven that the service is sustainable in terms of financial stability, that supports the current workforce.
